Patient Gateway is a secure portal for patients (and parents) to view portions of their children’s electronic medical record. For example, you can access test results, future appointments, medications, immunizations and summaries of previous visits. If you have misplaced your child’s health form, you can print out a copy in Gateway.
Enrolling in Patient Gateway is easy! We have provided the form to download. Once you complete it, please return it to our office via mail, fax or in person. For children under 13 years of age, the parents can access the information. However, for children over the age of 13, parental access is limited in concordance with privacy laws.
